Hi,
it is a new experience to me that the CBS changed their policy to have the Studies in Mycology online to everybody. Some times before this link http://www.cbs.knaw.nl/index.php/studies-in-mycology/341-studies-in-mycology-no-42 did lead really to the "full text" of the paper.

The CBS site has been renewed, but this resulted in the (deliberate?) loss of some useful web pages like the older issues of Studies in Mycology (recent issues since 2005 can be found at http://www.studiesinmycology.org; cybertruffle provides up to volume 30).
The download made available by Guy does not include the introductory paragraphs (p. 1-15). I can provide it separately if needed.
The very useful anamorph/ teleomorph database has also stopped working although the database form is still online: http://www.cbs.knaw.nl/databases/anateleo.htm. I have asked about this but I got an unsatisfying answer.
